Cybersecurity Breach at Vendor Compromises Personal Data in SafeVoice Nevada School Safety Tips

CARSON CITY, NV — A nationwide cybersecurity breach at a critical software vendor has compromised personal information tied to approximately 10% of tips submitted to Nevada’s confidential SafeVoice student reporting portal, state officials and the Las Vegas Sun confirmed.
The breach—detailed in formal joint disclosures released Thursday, October 22, 2026, by the Nevada Department of Education (NDE) and the Nevada Department of Public Safety (DPS)—originated inside the systems of third-party vendor Navigate360, which powers the backend tip-management platform P3 Global Intel.
The system serves students, families, and faculty across the state, including the Clark County School District (CCSD), allowing individuals to submit anonymous reports regarding school threats, weapons, self-harm, severe bullying, and student mental health crises.
According to state officials, an unauthorized actor gained access to the software vendor’s national environment.
A forensic audit conducted by cyber defense firm HaystackID established that roughly 10% of SafeVoice Nevada reports containing identifying personal information were exposed during the security intrusion.

Delayed Disclosures and the Threat to Student Anonymity
SafeVoice was established by the Nevada Legislature in 2017 under Senate Bill 212 and deployed statewide in August 2018 to ensure students had a safe, 24/7 lifeline to report dangerous circumstances without fear of retaliation.
While tips are submitted anonymously by design, users occasionally include identifying details, including their own names, phone numbers, email addresses, or specific descriptions of classmates and educators in distress.
State disclosures reveal that Navigate360 first experienced an intrusion into its P3 tip engine earlier this year, an incident that prompted congressional scrutiny from U.S. Senators Maggie Hassan and Jim Banks, alongside an active federal criminal inquiry by the U.S. Department of Justice.
State education officials pushed back against the vendor’s timeline, noting that Navigate360 provided incomplete forensic disclosures to Nevada regulators in early October.
Supplemental datasets were handed over between October 9 and October 15, enabling NDE to evaluate the scope of exposure and issue public warnings on Thursday.
“SafeVoice gives Nevada students a way to speak up when they are concerned about their own safety or the well-being of someone else, connecting those concerns with adults who can step in and help before harm occurs,” said Dr. Victor Wakefield, Nevada Superintendent of Public Instruction. “That trust is essential, and the compromise of sensitive information is unacceptable.”
Because SafeVoice does not collect mandatory account information from users, state administrators confirmed they cannot directly notify individual minors or parents whose records were compromised.
State Evaluates New Platforms and Establishes Helpline
Despite the vendor breach, state officials stressed that the SafeVoice reporting network remains fully active and monitored around the clock by trained DPS communications dispatchers.
School districts are advising families who believe they may have included identifying details in past tip submissions to consider credit monitoring services and data protection protocols.
